I have full understanding that this is a request form for my event to be hosted at The Epic. All requests will be processed and an event coordinator will contact the event organizer within 3-5 business days. Event request forms do not book or reserve a room or time. All reservations will be through an event coordinator after application has been processed. Only scheduled appointments will be honored.
